I understand that selection is not guaranteed and that I must complete any required SafeSport, background-screening, and Umoja travel/safety requirements before serving.

I understand that travel-expense coverage or reimbursement depends on the trip and assignment and must be confirmed by Umoja in advance.

Keep sensitive information out of this form. Do not submit Social Security numbers, bank details, passwords, driver's-license scans, background-check information, or medical records here. If Umoja needs protected information, staff will tell you where to provide it.
